Waiver

 1. That I am participating in the Martial Arts, Health, and Fitness Lessons, Practice, Programs, or Workshops (herein after called “The Activities”) offered by Granite Forest Dojo LLC during which I will receive information and instruction about health and fitness. I recognize that fitness programs require physical exertion that may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.

2. It is my responsibility to consult with a physician prior to and regarding my participation in the The Activities. I represent and warrant that I am physically fit and I have no medical condition that would prevent my full participation in the Activities.

3. In consideration of being permitted to participate in The Activities, I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I might incur as a result of participating in the program.

4. Granite Forest DOJO, LLC and its assigns reserve the right to use video and audio recordings, photographs, artwork, or other likenesses of Student for marketing, trade, training, publishing or any other lawful purpose, including social media and other internet venues.

5. In consideration of being permitted to participate in The Activities I knowingly, voluntarily, and expressly waive any claim I may have against Granite Forest Dojo LLC, instructors operating on its behalf, any organizations with which it, its members, or its instructors are affiliated, any organizations from which the instructors have received instruction, and the owners/operators of any facility in which The Activities are conducted, for injury, damages, or death that occurs before, during, or after participation in The Activities.

6. I, my heirs or legal representatives forever release, waive, discharge, and covenant not to sue Granite Forest Dojo LLC, instructors operating on its behalf, or any other organization from which they’ve received instruction, or the owners and operators of any facility in which The Activities are conducted, for injury, damages, or death that occurs before, during, or after participating in The Activities.

 

I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.  (By signing, any and all parties certify that they have read and understand all obligations, have received a copy of the agreement and are legally bound to it.)
